The very basic template file includes System, Design, Process & Machine FMEA, plus their suggested valuation catalogues.

You should only enter text to one cell at a time. The objective is not to have empty cells, therefore if a "function" expressed fills one cell only, but there are multiple rows for failure information, then the user should merge all the empty cells below that function accordingly. Failure to do so results in a loss of clarity and will cause problems, for understandability or when importing into other software product at a later date.

Examples are provided in the form of a spreadsheet¹ or a simple screen dump².

The QS-9000 3rd Edition template shown above has now been superseded by the AIAG version in March 2008 (QS-9000 4th Edition). All of the latest versions of the FMEA form sheet layout are available in the latest version of the software that we recommend.
